Clinical presentation:
Indian lady with abdominal pain.

This is a series of barium follow-through views of the ileo-caecal region taken at roughly monthly intervals. They show a narrowed segment of terminal ileum draining into a narrowed caecum and ascending colon. Initially, the nodular mucosal thickening in the ileum is marked, but improves with time. The colonic lumen becomes wider and less distorted, but there is some shrinkage towards the liver.

The appearance of colon shortening is unusual, unless related to Surgical resection. Most of the cases of colonic fibrosis in this database such as Scleroderma or Laxative abuse have redundancy rather than shortening.

Caecal volvulus. (shown left) may perhaps imitate this appearance, but the obstructive element is obvious.

diverticulosis is usually descending and sigmoid colon.

Infarct or ischaemia are more probably found in the region of the splenic flexure, but listen to the heart valves before excluding the possibility of embolus.
