Clinical presentation:
Adult female with chronic heart condition and recurrent cough.
The heart is large with a straight left margin and a double contour. The density of a enlarged left atrium can be seen with widening of the sub-carinal angle (between the main bronchi). The aorta is not enlarged. The lungs are normal size, but show a complex appearance. There is pruning of the main pulmonary vessels as they leave the hila. Scattered nodular shadowing is present in the mid and lower zones. The bronchial markings are also increased in the lower zones and the left mid-zone with some dilatation. In the left lower lobe the main bronchial lucency can be traced into a branching density that has the inverted gloved hand appearance. The shadowing is confluent at the right costo-phrenic angle with blurring of the diaphragm margin and the horizontal fissure is widened.
